Recommended Access to Information for the audit

Access to the following information would be beneficial to ensure the audit is as informative and successful as possible.
The order of access is not important. Brief access may be necessary to clarify specific issues and provide an accurate assessment.

The following is a sample of information that may be required depending on which type of audit is being performed
(specific information will be discussed in more detail at the pre-audit scheduling meeting):

1)   HR files, including organizational chart, job descriptions, training records and materials, etc.

2)   Maintenance Management programs (PM, PdM, Process Trouble Shooting, Repairs, Parts Control, etc.),
       schedules, records and forms with subsequent access.

3)   Sample

4)   Process trouble shooting procedures, repair/major overhaul procedures and related reports

5)   Sample shut down job packages for complex critical equipment

6)   Reports and Investigations Corrective/Preventative, QA, QC, repair, etc.

7)   A tour through the information system (capacity/potential versus actual usage) with subsequent access.

8)   Assistance by a team member educated about the computer management system, required spare parts
      and who determines and how min/max inventory levels are determined

9)   Access to various manuals (Procurement procedures, Maintenance and Parts Control Quality, PM manuals
       for major, minor (critical and non-critical equipment), Repair/overhaul, training)

Typical Pre-Audit Meeting Agenda

1. Define any safety, security, training/orientation requirements for Carver PA Corporation Auditors

2. Define any security or confidentiality boundaries (e.g. documentation-what can or cannot be removed from site)

3. Define where the auditors can perform the interviews and store reference documentation for the entire week.

4. Review organizational chart and define department contact names, phone numbers and locations

5. Review audit proposal content for clarification, specifically ACCESS TO INFORMATION

6. Define who will be the best person to contact to obtain the information outlined in the above document.

7. Define key people who have time constraints

8. Define client meeting schedules/constraints and Carver PA Corporation attendance

9. Discuss how and with whom the interview schedule is to be arranged (the interview schedule is flexible
   and open to last minute changes if required).
